Application for Sponsorship

Please apply ONLY IF your pet’s mobility has been affected. For help with care not related to mobility, please visit our Resource Page. At this time, we are only able to provide rehabilitative and/or veterinary care to pets in the Chicagoland region.

In order to be considered for sponsorship the following must be received:

1. The completed application form below.

2. Your pet's recent medical records supporting the request for sponsorship.

   NOTE: A medical record is a record of a patient's health and medical history including a list of diagnosis or differential diagnosis by your veterinarian(s). You must request these documents from your veterinarian(s). Please do not send us invoices.

3. Recent Video(s) of your pet gaiting (walking).
   
   NOTE: Please email medical records and video to: application@gowildhearts.org if you cannot upload via the links below.

PLEASE NOTE: At this time we are not able to assist with requests for:

• immediate/urgent need

• surgery

• common orthopedic conditions such as arthritis, spondylosis, ligament tear, luxating patella or dysplasia

• reimbursement for invoices already paid
